Rejected NDIS claims cost you more than lost revenue. Each one triggers a rework cycle that eats into your billing team’s time, delays your cash flow and, under the new digital claims system rolling out from July 2026, creates a pattern that attracts scrutiny.
Most rejections come down to the same handful of NDIS billing mistakes. They’re avoidable once you know what to look for. Here are the seven that trip up providers most often and what to do about each one.
At a glance: The seven most common NDIS billing mistakes are wrong support item codes, exceeding the price limit, claiming outside the plan period, mismatched participant details, duplicate claims, claiming for services not in the service agreement and billing before the service is delivered.
The 7 Mistakes
- Claiming under the wrong support item code
- Exceeding the price limit
- Claiming outside the participant’s plan period
- Mismatched participant details
- Duplicate claims
- Claiming for services not in the service agreement
- Billing before the service is delivered
1. Claiming Under the Wrong Support Item Code
This is the single most common reason claims get rejected. Even a minor code variation, like using a general Core line item instead of the correct Home and Living line item, causes an instant rejection.
The 2026-27 Pricing Schedule made this harder by splitting several categories into more granular codes. Cancellation, telehealth, provider travel and report writing are now separate line items instead of modifiers. If your system still generates claims using the old modifier structure, every one of those claims will fail.
The fix: Make sure your billing software maps services to the correct support item codes automatically. Default Service Items pre-fill the right code for each service type, so your team reviews claims instead of guessing codes.
2. Exceeding the Price Limit
Your claimed rate must match the NDIS Pricing Schedule or the rate specified in the service agreement, whichever is lower. Even small discrepancies flag the claim. A rate that was correct last month might exceed the new limit if you didn’t update after 1 July.
The fix: Your software should pull in updated price limits automatically when they change. If your team manually updates rate cards, errors are inevitable. Automate the update and remove the risk.
3. Claiming Outside the Participant’s Plan Period
Claims referencing dates outside the participant’s current plan period fail every time. This happens when plans expire, when a new plan starts on a different date than expected or when your system doesn’t have the correct plan dates loaded.
The fix: Your system needs live visibility into each participant’s plan period. If a session falls outside the plan dates, the system should flag it before the claim is generated, not after it’s rejected.
4. Mismatched Participant Details
Typos in participant identifiers are surprisingly common and cause instant rejections. A wrong NDIS number, an incorrect date of birth or a name that doesn’t match the NDIA’s records will bounce the claim before it’s even assessed.
The fix: Enter participant details once, verify them against the NDIA portal and let your system populate them automatically on every subsequent claim. Manual re-entry on each invoice is where errors creep in.
If your billing team is spending more time correcting rejected claims than submitting new ones, it’s worth looking at how your current system handles these checks. Book a quick walkthrough with FlowLogic to see how pre-populated claims reduce errors at the source.
5. Duplicate Claims
Using the same invoice number, or submitting a claim for a session that’s already been paid, results in automatic rejection. This happens most often when billing teams batch-process claims and lose track of what’s already been submitted, especially during busy end-of-month periods.
The fix: Your software should flag duplicate claim numbers and duplicate session dates before submission. If your billing team has to manually check for duplicates in a spreadsheet, the process is too fragile.
6. Claiming for Services Not in the Service Agreement
If your invoice lists a support category that isn’t in the participant’s current service agreement, the claim gets rejected. This happens when service agreements expire, when participants change plans or when providers add new services without updating the agreement first.
Claiming provider travel without prior agreement in the service agreement is a common version of this mistake. Travel is claimable, but only if it’s documented and agreed upfront.
The fix: Link your claims to active service agreements in your system. If a service isn’t covered by the current agreement, the system should block the claim and prompt your team to update the agreement first. Keep your service records, agreements and staff credentials connected so nothing falls out of sync.
7. Billing Before the Service Is Delivered
NDIS funds cannot be claimed until after the service is delivered. Generating and submitting invoices for scheduled sessions that haven’t happened yet will get the claim rejected and may trigger a fraud flag under the new Integrity and Safeguarding Bill.
The fix: Only generate claims from completed service records, never from scheduled shifts. Your software should enforce this by only allowing claim generation after a session is marked as delivered with supporting notes.
Why This Matters More in FY27
The new digital claims system cross-checks every claim against your service delivery records, participant plan details and provider registration data. Under the old system, a rejected claim was an admin headache. Under the new system, a pattern of rejected claims can trigger an investigation.
From 1 December 2026, the claim submission window drops to 90 days. A rejected claim that takes two weeks to rework is two weeks closer to that deadline. Multiple rejections in a month can create a backlog that’s hard to clear before the window closes.
Getting claims right the first time isn’t just good practice. It’s the difference between getting paid on time and chasing rejected claims while revenue sits in limbo. We covered the full claims system overhaul in our post on what NDIS provider software needs to handle in 2026.
Ready to Fix Your Claims Process?
If your team is losing time and revenue to rejected claims, the problem is almost always in the system, not the people. The right software catches these seven mistakes before they reach the NDIA.

Frequently Asked Questions
What is the most common reason NDIS claims get rejected?
The most common reason is claiming under the wrong support item code. Even minor code variations, like using a general Core line item instead of the correct Home and Living line item, trigger instant rejection.
Do I need to update my billing system for the 2026-27 Pricing Schedule?
Yes. The Pricing Schedule introduced new standalone line items for cancellation, telehealth, provider travel and report writing. Claims using the old modifier structure will be rejected from 1 July 2026.
What happens if I submit a duplicate NDIS claim?
Duplicate claims are automatically rejected. This includes claims with the same invoice number or claims for a session that has already been paid.
Can I claim for provider travel under the NDIS?
Yes, but only if travel is documented and agreed in the participant’s service agreement. From 2026-27, travel is claimed at 50% of the therapist’s hourly rate with time limits of 30 minutes in metro and 60 minutes in regional areas.
What is the 90-day claim window?
From 1 December 2026, any support delivered after that date must be claimed within 90 days. Rejected claims that need reworking eat into this window, so getting claims right the first time matters more than ever.
Can billing before a service is delivered trigger a fraud investigation?
Yes. Under the Integrity and Safeguarding Bill (April 2026), submitting claims for services not yet delivered can be treated as fraudulent claiming. Only generate claims from completed, verified service records.
